Depending on the medical insurance plan, the patient may be eligible for custom orthotics (arch supports, etc.), certain durable medical equipment/appliances (walking boots, braces, splints, etc...) and diabetic shoes.

- If you would like to know if our doctors are on any of your health plans, you may call the office at 562-597-5100 to inquire.
- The office staff will verify your medical insurance for eligibility and benefits. The patient will be responsible for co-pays, deductibles and all fees not covered by their insurance plan.
- HOUSECALLS can also be arranged as the doctor sees medically necessary, if the patient has MEDICARE PART B or pay a nominal fee.

All patient balances are due upon receipt of bill or on next office visit. All co-pays are collected at time of service.
